EASTERN SUB SERIES BJJ COMPETITION

      RELEASE AND WAIVER OF LIABILITY                                                                                                                                                 

In consideration of being permitted to participate, in any way, in the Eastern SUB SERIES BJJ COMPETITION, (the “Tournament “), I hereby agree that:

1) I understand the nature of Brazilian Jiu Jitsu activities and the rules of Brazilian Jiu Jitsu sport competition, and I believe that I am qualified to participate in such activities.

2) To the best of my knowledge, I am in good physical condition and have no disease or injury that would be aggravated by participating in the Tournament, and that I do not have any diseases that would be communicable to others.

3) Prior to participating in the Tournament, I will inspect the mats, equipment, and facility to be used for the Tournament, and if I believe anything to be unsafe in any way, I will immediately inform my coach, instructor, or tournament official of such conditions and refuse to participate.

4) I acknowledge and fully understand that Brazilian Jiu Jitsu sport competition is a contact sport and includes throws, chokes, and joint manipulations, and that these activities might result in serious injury, including permanent disability or death.

5) Knowing the risks involved in Brazilian Jiu Jitsu sport competition, I assume all such risks and accept personal responsibility for the damages that may occur from my participation in the Tournament.

6) I release, waive, discharge, and covenant not to sue PERKINS JIU JITSU, HAMMERS GYM, their respective associated Gyms, administrators, agents, coaches, referees, scorekeepers, medical staff, and any other employees or volunteers, from any and all claims, demands, losses, or damages on account of injury, including permanent disability, death, or damage or loss of property, caused or alleged to be caused in whole or in part by the negligence of any of the aforementioned persons or corporations.

UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND KNOWING THIS, I

SIGN IT VOLUNTARILY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE. I

AGREE TO PARTICIPATE IN THE TOURNAMENT KNOWING THE RISKS AND CONDITIONS

INVOLVED AND DO SO ENTIRELY OF MY OWN FREE WILL. I AFFIRM THAT I AM AT LEAST 18

YEARS OF AGE, OR IF I AM UNDER 18 YEARS OF AGE, THAT I HAVE OBTAINED THE REQUIRED

CONSENT OF MY PARENT OR LEGAL GUARDIAN, AS EVIDENCED BY THEIR SIGNATURE BELOW.

I INTEND THIS TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABLITY TO THE

GREATEST EXTENT ALLOWED BY LAW.
